Registration for the meeting is available until the start time of the teleconference 1:00pm ET December 4, 2008. You must register on-line the day of the teleconference - no phone or fax registration accepted after December 3rd.  Please register early to avoid delays in processing your registration information.

To protect your credit card security we are accepting credit card payment ONLY through online registration.  If you wish to fax/mail your registration and pay by credit card, please call the JCEB to provide your payment information 202-662-8676/8640. 

JCEB accepts the following credit cards as payment for registration - Mastercard, Visa, and American Express. Please note that you must provide the 3 digit security code found on the back of your credit card for credit card processing.  (The security code is a 4 digit code on the front of the American Express Card.)  You must also provide the billing address for the owner of the credit card. 

To pay by check, please mail in your registration form with payment.  If you are mailing your form less than one week before the teleconference, please fax a copy of the form and check to 202-662-1012 to make sure you receive the proper information in time to participate.
